I am using the form block to take input from the end users and giving an option to upload a file . The file being uploaded from the form is in stored in file manager but seems like its broken. we are not able to view the image .

What version of concrete5 are you using?
Have you modified any core files?
Does this only happen with image thumbnails uploaded with the Form block? Are you able to view image thumbnails when you add the images through the file manager?
Are the other thumbnails generated for your image. Thumbnail images are available through the left click menu in the file manager and image selector form widget (Thumbnail Images or Properties > Thumbnails).

I am using Concrete 5.7.5.6 .
Havent Modified any core file.
Able to add through file manager , but when adding through form block facing this issue

Are you able to download the uploaded image through the file manager by right clicking the image and selecting "Download"?
When you say the image is broken and cannot be viewed, can you take a screenshot of this please.
