Wufoo Woes

I am building a sitewww.www.allgodschildrenministry.org... and was not able to get the form to work. Even the simplest form would not submit. The form would just clear, no data went through, no email sent. I gave up and added the wufoo app, built a form through them and added the link as an external link to the navigation across the top under Contact Us. It has crashed the site. I am getting the error

Spoke with Wufoo at length. Although this is listed on Concrete5 as a 5.7 Add-on, it does not work with it. I was able to create forms through Wufoo and add them as an external link. The external link in 5.7 is currently buggy, but if you search for external link bug you can find that solution.
