form is not appearning when I am dropping my block to page

Hello,
I have developed an addon in which I have a block, named as Lead captured form. When I drag and drop the block on the page, the progress line on top disappears after little progress and nothing work onward as expected. It should open a form added in add.php in a popup window.
Can someone guide me what is wrong I am doing?
PS:: I did develop 3 other blocks the same way, those are working perfectly fine.

Sounds like you have a PHP error, are there any errors in the developer tools console of your browser when you add the block?
