Can't add new page

I can't add a new page. The window with "choose a page type" is empty (see attached image). But if I try to change the design of a page, the available page types are shown and changing works without problem. Editing, moving, change order -> no problem too.
(c5 version 5.3.3.1)

Did anyone had the same issue?
Any ideas?

I just figured out that if at least one "custom fields" (in properties) is selected, regardless of its values, the "add new page" is not working. In other words: If all custom fields are removed, I can add a new page below that specific page.
This smells like there is some kind of left over from an earlier version?!
Very strange ... keep trying to solve it.

when turning on PHP/DB debugging, I'll get this:

Actually I could hit myself ... (don't trust the smile in the picture ... currently)

There was an old version of "collection_add.php" (v5.3.2) in the root folder /elements which cause the trouble.
It was a customized version where spaces were replaced with "-" instead of "_".

This is the default method anyway now...
