Captcha not accepting correct code

We are experiencing an issue with new user registration that is caused by the captcha not accepting the correct code. I have multiple reports (including screen shots) and have reproduced the problem myself, but it seems that not all users are experiencing the problem as we still receive new registrations daily.
Can anyone shed some light on this?
