capturing email addresses

I would like to offer visitors to our website the opportunity to download a free Ebook I've written in exchange for completing a simple form with their first and last name + email address and I would like to verify that the email address is correct prior to them being allowed to download the book. I have the Ebook saved in pdf in my files. How do I do that?
