data capture form on landing page

Is there an add on where you can have a data capture form as the landing page of a site. Or can it be done by some how making the landing page a form that needs to be filled in before you enter the site. This is not to register or login, but to certify the browser.
When you mention "certify the browser", what are you trying to do?
