Look at the source code for the rendered page and check for an error message. Since the code that creates the edit bar is one of the last things generated, any error on the page that stops DOM processing will prevent the edit bar from displaying.

Thanks for your help. I wasn't aware error messages could be generated in source code? I had a look and no sign of any. I solved the problem by rebuilding the pages which has worked.

however I now have this on another site. The edit bar disappears when you navigate to it from another page but appears when you go to the page from sitemap. very odd. Rebuilding it this time hasn't worked.
