Form Results saved a .csv

I am considering using Concrete5 for maintaining a tourism web site. I need to develop a form for users to request a brochure from the web site. It will collect their email, mailing address, and activities that they are interested in, etc. I want to email the form results so that it can be printed and given to someone to fulfill the request. I also need it to save the results to a .csv file. Is there a way to do this or an add on available? I am planning on using the Data Display Add On (For listings of restaurants, shopping, etc.) and wasn't sure if I can use it for this somehow. I don't necessarily have to have it in a .csv file, I mainly need a way to print out the results in a report to be used for grants and marketing.

Well you can certainly access the data directly in the database. I use Sequel Pro on a Mac. With that I can get right into the db and find the from data and export it in a variety of formats, including csv.

Then, of course, it is a simple matter to make an excell spreadsheet or text file for someone.
