Form sends email twice

Hi

I've used the Extended form add-on on this page:
https://www.countrysidenurseries.co.uk/contact/prospective-parents...

But whenever anyone submits it, it gets sent twice to the recipient email address.

Does anyone have any ideas why, or how to fix this?
