tracking file downloads from our website

We want to post links to free educational materials on our website (fresnoartmuseum.org) but we want to be able to track anyone who downloads the files by capturing their email address and affiliation (i.e., parent, school, museum, etc.) We want this so we can get a sense of who and how many are using our materials and for possible follow-up surveys sent via email. I am not a developer so I need to know the simplest way to do this on our Concrete5 site.

I used this add on for a client to capture visitors email address for file downloads.

My client finished up setting it to send them the download link as some site visitors were entering fake addresses.

You can also use the basic contact form. The redirect to another page lets you send users to a new page after submitting it. If you want more control over the thank you message, this is a great way to go. You could also build a "fill out this form to get access to our..." type deal with this. There's a couple of great marketplace ad-ons too!
