Add-in licence "lost" after rebuild

I rebuilt a site that had been infected with viruses at Dreamhost. I've tried several times to re-install the Accordion menu to the new "incarnation" but am having no success. It shows up as an available install, seems to install correctly, but then I am not able to create the block.
