Add Options/Manage Attribute Types Issue

I'm hoping someone can tell me what the problem is here. I've added a new product in Core Commerce, added images to it and then tried to add a customer option under Select-Product Price. When I click add attribute, I get some weird screen (see attached file) and can't do anything. The option does not get added.

Ideas?
