Add Page button missing from Sitemap

I a have weird problem after upgrading to 5.4.2.
I just tried to add a new page to our site and the add page popup window is missing the Add Page button completely.
See the attached screenshot.
I have browsed this form and found some possible solutions. For example wrong database table names but those do not seem to my case.
