Adding a static form block to the template

Does anyone know if this is possible? If so, how did you do it?

FYI, the reason I want to do this as a static block is that I have a custom attribute on a page type for email address. I'd like to have the that page type automatically include a form with that email address as the reply-to address.

You could just add the form block to the default Page Type and set it to appear on child pages.

As for automatically including the email address, you can make an alternate template of the form block to do this.

I'm not against this approach. What is a good approach to changing the recipient email address in a block template?
