admin user doesn't have "+add page" available, only "edit"

I am having an issue where an admin user is signed in, goes to the site and is only offered "edit" in the top control bar and not "add page". My admin account offers both.
I have looked at all page permissions and they have all permissions. I am at a loss of how to solve this.
This user can add pages from the sitemap...weird

they get the "edit" option, but not "add page"
Using advanced permissions?
Try under the set permissions tab, the dropdown box "sub-page permissions".
Select there which page types to use and then you are all set.
This should however not effect the super user (user created upon installation)

I'm struggling to set a user as an "Approver", someone who can approve and publish pages that are created/edited by other authors.
Can't find such an option anywhere.
Is "Advanced Permissions" an add-on that I need to install? If so, where can I get it please?
