How will this be affected when you upgrade?
Does it just overwrite the files?
I'm concerned that if I add it to a client's site and they click the upgrade when it's next released...nothing "bad" will happen.

when you upgrade you have to re apply the patch

Update for 5.4.1.1 (Advanced permissions )
Use the new 'include file manager to edit bar' function from the dashboard, Sitewide settings, Editing Preferences.
Then add your user or group and make sure they have the right permissions to access the Sitemap and the File Manager.
Go to the Dashboard, in the Sitemap Page, under the Access tab and add the necessary permissions.
Add your user or group, don't worry about 'Access Sitemap and Page Search', just say yes. then save....
The same goes for the File Manager: Dashboard File Manager Access tab...
Add your user or group, tick want you want to allow and save.
