Best way to approach this in C5?

I have an upcoming project that may need a report builder like this one (http://www.choosecincy.com/reports). Is there a way to do something like this with tools or add-ons in C5?

theres the form/survey block, and some more advanced form addons in the mp.

I've been looking into those add-ons but not sure how it can take blocks and group them into a report. Any suggestions?
