Can't insert form

hi,

I'm currently building my 3rd site with Concrete5 and I'm having problems inserting a form onto my contact page. The add form box comes up just fine, but when I click 'add question' nothing happens, no error but no question added either, just nothing. I have successfully added forms to my previous 2 sites with no problem. I am running the latest version of C5 on a linux server.

Can any one help please because I've tried everything I can think of!!

As an extra point. My other sites are both running on the same server and with the same version of C5 and I can add forms without any problem to those so it appears to be something site specific.
