I lost my contact page and the link to it in the breadcrumb bar. I didn't see an add on I could use to get it back on. Any suggestions on how to get a contact page with accompanying fields back on my site?

What contact page are you referring to? I don't think concrete5 comes with a built-in contact page, so this was something that was added by you (or whoever worked on the site before you).

I think the easiest thing to do is just add a new page to your site and put the Form block on there, configure the fields you want in the contact form, and set it to email you when someone submits it.

I'm having the same problem.

I've been having C5 add sample content when I install it. On the first site I did there was a contact form on the About page already created. I did the second install the same as the first and had C5 install sample content, but this time there is a Blog page and no contact form on any of the pages.

I'd like a contact form on the second site as well and can't find any reference to it in the add-block selection window or anywhere else.

Jordanlev, if there's supposed to be no built-in Contact Form then where could it have come from on my first install?

Is there any way I can copy the contact form from the first site into the second?

The contact form is just a "Form" block. Just add a "Form" block and configure it to have "Name", "Email", and "Comment" fields (or whatever you want in your contact form), and have it send you an email notification when someone submits the form (see the "options" tab over to the right when you're editing the block).
