Custom Form That Does Calculations

I have a client that works in real estate and wants to pull up what essentially is an app from his website to do calculations and send them to himself and associates. Here is a break down of his process:
1. Go to potential property and pull up the real estate website and visit his custom calculator / form.
2. Fill it out. (the form will do some addition / subtraction calculations based on the the answers from the form).
3. After hitting submit, the form's answers and the calculations will be sent to a group of investors to determine if the property is worth it or not. I've attached screenshot of the XLS file that was sent to me to model this form after.
Is there already an add-on for this? I figured it will be a custom form, but any guidance would be great.
