Display data from form fields

Hi,
I need to be able to list the inputs from a form block. The function I need is a petition list. Think it is quite easy... but I am stuck. Can anyone point me at the right direction? Like the syntax for loading the data. I can see that that the table I need to access is btformanswers and the field the row that has a special msqID
Been looking at model files, controllers... etc.
I suck at php...
Any help is greatly appreciated!

Why not just use the Survey block? You can easily manage such a thing without having to write any code.
I can only choose radio boxes with the survey block. Is there a way to add text fields?

Hi. The Display data add-on is amazing. It's packaged with an advanced form builder too.http://www.concrete5.org/marketplace/addons/data-display1/...
There's a how to screen cast for the advanced form builder, however the documentation of display data is still for the older version.
$150 is good value considering how much it would cost to hire a programmer.
