Dynamic Survey Form Name on Page Type Template

Hi !
I am struggling to create a page type containing a survey form with dynamic name depending on the CollectionName. My clients are comedians and they need some sort of reservation solution. I figured I could create an "Event" page type - so that they can easily fill out the info with a composer - and add a Survey Form in the page type.
The problem being that the survey form has only one name, which means that in the Report page in the backend, all the various events' reservations are stored within this same survey report.
Do you have any suggestion for a workaround ?
