External form template not working

Can anyone show me how to code an external form template so my forms are emailed rather than written to the db?

I tried making one but when I try to bring it into the page, everything freezes.

I have tried code that works outside C5 but when brought in just breaks.

Any ideas please?

Attached is a sample External Form that only sends the information via email and does not save a copy to the database.
