Form Address Block Issue

Is anyone having issues with the new forms in 8.0+? Specifically at times users browsers will not populate U.S. States, therefore making it impossible for them to finish a form where Address is required.

The states are populated with javascript, so if your site has javascript errors that could break the functionality. Do you have a link to the page where this is happening?

That page has this error that is likely causing the page to not function

Uncaught TypeError: $(...).responsiveSlides is not a function

Do you know how I could fix this? Would this be a Theme or Concrete error?
