Form block contact form having a few problems, trying to figure out best way to fix it.

Using c5 5.6.3.1 have upgraded many times from previous versions.
Not sure when this started but my form block is having problems such as,
1. Thank you message not appearing after hitting submit.
2. Not saving the changes for the checkbox "reply to this email address"
3. And the Main problem, emails sent from the form do not contain the input form data.
Not sure a good direction to take to fix these, other than uploading the concrete5 files again, is that a good way to fix this?
