Form to pull out data from Mysql database

Hi all,

I have a MYSQL database hosted in my domain and I would like to connect it to a form which would allow my visitors to query the DB and display data from it.

I have found this thread that will help me to connect to DB.

Any advice what steps to follow? What would be the best way to do this in Concrete5? I am not pretty sure how to connect the form to the db.
