Form with 50 plus checkbox's

Has anyone tried to add a large number of checkbox's and got clear results on the extended form - I'm thinking of buying it as the standard form gets in a muddle and the extended form generates field ID's which should help.
Any feedback appreciated

Try to get in touch with the developer and see if you can get a login to play around with. Click on Pre-sale questions on the right:
