You can check the documentation of the add-on. Its described very clearly there. You will find that to the right side of the add-on details page.

Check this link:
http://www.concrete5.org/marketplace/addons/internationalization/do...

Basically you need to add the languages that you want to have in your site from dashboard.

Suppose you have 2 language. So the sitemap will be like
Home
-En
--Page1
--Page2
--......So on
-Gr
--Pg1
--Pg2

Like that.

So you mean,

manually create in same page in different language? right?

for ex:

i was create about.php page in my website in english.
if i want to change about.php page in another language like in Chinese
mean i create about.php in chinese language also ah?

else any other solution in concrete5

No you are not right. Suppose you want to create a page called "About Us" under English. So create that page & then go to dashboard Internationalization. There you can find an option called Copy Tree. This will copy the page from one language to another. You need to choose From English tree to Chinese tree.
