Hyperlinks not including WWW

Seemingly sporadically, hyperlinks to files and pages do not contain WWW and the link fails for users. I have tested this on ALL browsers, MacOS, Win8 & Win10.

Trying to figure out if this is a server issue: we have an internal win 2012 server or if this is a concrete issue.
