Initial Page Load Always Requires Refresh

This is really weird. I've developed websites on several different CMSs, but I've never had this happen before. Whenever I go to the site the first time, nothing loads. It's a white page. There is no HTML, no response from the server.
When you refresh, the page will load, and all subsequent loads will work until you clear your cache.
This has happened with the same sight on three different servers, so I don't think it's a server issue. I think Concrete is either timing out or not sending a response to the initial request.
Any thoughts? This is killing me, and it's happening on a live site none the less.

Can you go to Site Wide Settings > Debug and the paste what's in the environment box here?
If it's occuring when you clear your cache my initial thought would be something to with cookies ....
Just thinking out loud here ...
