Insert Image Between Form Fields

I would like to insert an image between form field items. The structure would look something like this:

Form Field 1
Form Field 2

Image

Form Field 3
Form Field 4
Form Submit Button

Any ideas or suggestions would be appreciated.

If you are using the standard form block that comes with the core, then one way you could do this would be to create a "custom template". There is a forum post on how to do this:

That link gives generic instructions for making a custom template for any block. I am looking for something specific to the forms block. Any targeted advice, or links would be appreciated.
