jcart shopping cart to email

I have built a small site using the free (old) jcart shopping cart. I can get the values form the cart through to an email (rather than the default PayPal) but don't know how to also have a form in the gateway to have customers add their details then email through with the shopping cart values.

PS Please suggest coding rather than using a theme or Core eCommerce as I need a quick responsive solution.
