Need help implementing pagespeed/yslow recomendations on slow site

I'm hoping to get some help on speeding my site up. Right now we are on a vps with dreamhost, and have a cloudflare plus account activated.
I ran pagespeed and it came back 81/100, ten minutes later I ran it again and it was 72/100. I didn't change anything in that time and am not sure about why it changed. I have noticed though that the site speed is not consistent. Sometimes it seems to work great, other times it barely crawls along.
On yslow
the site has an F grade for "make fewer http requests". "Add expires to headers", "Compress components with gzip", "Put javascript at bottom", "reduce the number of DOM elements", use "cookie free domains"
