Online form to populate an online profile page

I manage a non profit softball team/organization website using concrete5 and I was wondering if there is a way to have players/parents fill out on online form and have that information once the form is submitted create and populate an online player profile page. Thanks for your help!

You can create this by adding user attributes for each of the elements you want to show and configuring those attributes to show/require on the individual profile page.
