does anyone know of an add-on that would allow me to create an online form such as a clothing order for one of our fundraisers? we do own formidable and that does a fairly decent job to collect data but I need to be able to perform calculations and create a unique order number for the buyer to reference.

any information or suggestion would be appreciated....
