Paging broken, page 2+ will not show and ccm_paging_p_ seems to be random

All my paging seems to have broken.
The string that comes after ccm_paging_p_ in the URL changes on each page load - is this right? On other sites it stays the same, its almost like is is generating a new ccm_paging_p_ number each time.
Page 1 works, 2 and onwards just shows page 1 again.
What is wrong?
