What are your permissions on the files folder in the root of your installation?

If the account under which Linux is running the web server (often www-data) does not have the proper permissions, then it can not store the files. My folder has permissions of 755 (rwxr-xr-x).

Hope that helps.

Thanks TeachBlessing for ur reply. the folder has all necessary permission, but i'm talking about forwarding the the content of the form to an e-mail. In the mail i got all other parameter except the attach file. just want to know if there is any thing to set in other to receive the attach files.

Just so you know, you can get the file by going to Dashboard -> Reports, clicking on "View Responses" next to the appropriate form, and finding this submission in the list (you may need to click the little "View all fields" link under the submission you want to get the file for).

And your friend may be using the "Extended Form" addon from the marketplace, which I believe does handle this functionality (although it is not free like the built-in form block):
