Questions disappear on Form block

Hi all,
I'm new to concrete5 only been using it an hour or so, so far it's fantastic! I just have one problem, when trying to add a form, I add the questions but then when I click to add the form, they disappear and it just says "please add a question" for the form.
I'm using the standard theme, with no extra blocks or changes,
Any ideas?

how did you install c5? by a one click installer?
Yes I did, could this be the problem? Should I install manually?
often there are errors in the installation when using an instant installer, so it would be worth a shot to install manually.
