Slider Interface on Dashboard Single Page

I am trying to put an input slider on a dashboard single page as part of a form for a package setting.
I attached a screenshot of what I'm talking about.
I know how to do this with the form for a block on the front-end, but how do you make this work on a dashboard page? Can it be done?
