Submitting a Form goes to blank screen

So I have a custom form block and it is supposed to display a message when submitted. But instead, when submitted, it goes to a blank screen. Thus the user doesn't realize that it has been submitted. Any ideas?

Update: Realized it only goes to the blank screen when I have "Notify me by email checked".

If you check your logs, do you have any exceptions relating to email being listed? I imagine your site is in product mode, if you go into dashboard > settings > debug and choose "development" mode and re-submit the form, do you get any errors?

What version of concrete5 are you using? There was an error in the earliest versions of 5.4 where using multipel email addresses in the form block wouldn't work, even though the form block says that they should. This has been fixed in 5.4.0.5
