Toolbar Showing on Live Site & 'MiniSurvey' error

I am working with a copy of the Elemental theme. The cc-toolbar displays as it should when logged in but continues to be displayed when logged out.
Adding a form is also leading to the following error:
All content before the form's div is visible, but anything after it is not. No form is visible in the editor.
All existing Javascript remains untouched.
