Two Forms on Two Pages only 1 submission email is received.

I have created to form blocks on different pages of my site. When I submit the first form created I get the submission email. However, when I submit the second form I do not receive the email. Any suggestions?
If its the same block type, I would just assume the options for the second form are not correct. Perhaps the notify me by e-mail is not checked or the e-mail is not correct?
For some reason I never thought that it would not be checked. I actually clicked the box and entered an email address in the text box. Guess I missed the box when I clicked.
