URGENT: Receiving blank forms even though every field is required!

Hi
My site is running C5.7.3.1 and I am receiving blank form submissions (please see attached) despite making every field as required. (Core form block)
Also the logs don't have any information, not even the names of the fields. When I test the form logged in or not it works fine: shows all info in logs and comes through fine by email too.
Anyone else seen this issue or can recommend anything to check?
