Weird Error on adding a Survey in "Sidebar" Area

This happened when I added a survey with three questions to a testing site with a "Sidebar" area.
Not a big deal to me, it's just a testing site. But thought it might be a bug.

Notwithstanding, the survey was there after I went back to the page while still in edit mode.
thats usually a cache issue
