When to re-install custom block?

After I install basic_test, can I just edit the php/xml files to make updates and corrections? Do I ever need to do a re-install? If so, how?

No. AFAIK, you can just keep editing and improving your code without having to reinstall it.
Even when you update the db.xml file, you can refresh the schema from Dashbaord -> Site Settings -> Debug -> Refresh Schema
