I am wondering if anyone might have a solution to this task involving working with forms?

I am trying to have functionality whereby users choose a number of members at inception to be registered into the system. If say (1 member), the form fields display Name Address Email etc for only 1member entered. If 2members, then there should be a link to add more and the next fields show up to be filled out and so forth.

Is there any good add-on out there that anyone has tried and could help me? I'm not that great at PHP and would appreciate a simpler solution that I can understand and customize.
