403 Forbidden response when submitting form

I created a form on a new page. All that went well, but when I submit the form with data, I get a 403 forbidden response, you do not have permissions to access index.php. I'm on shared hosting, don't have pretty urls enabled. I'm guessing I have to set up an .htaccess file.

Any suggestions?
