Built in Form block dont work

Hi.
Cant use the builtin formblock to work, get error message:
Message: 'get(...).checked' is null or not an object

Char: 4
Code: 0

The problem occure when I try to add a question to the form. Can I reinstall the original blocks somehow?
Running version 5.4.0.5
PHP 5.3.2
But was the same with older versions.
