Can't access pages - get error Page Not Found

When I try to visit the home page, (directly or throught the site map), I get Page Not Found. No page could be found at this address.
Can not add a page either.
Maintainence mode is disabled, Viewing permission is public, refreshed the core database tables and blocks. Set the debug level to development (no errors in PHP or database are showing)
I am at a loss did everything the same as the last 4 sites I developed. Hosting company is the same as well.
Any ideas?
